Complete the Application Online. Or follow the instructions below to send the form to us via fax:
- Click on New Customer Application, complete information, print, sign, and fax back to us at 724-853-4049.
- The application must be completed in its entirety and signed by appropriate personnel.
- Credit analysis is performed for all new customers to establish standard payment terms.
- We must have a signed application and laboratory services quote on file prior to sample submission.
Sample Submission:
- Click on Resources in toolbar above for sample collection and shipping instructions.
- Download, print, and complete the appropriate Chain of Custody/Test Request Form.
